EUS guided Cystogastrostomy

EUS-Guided Cystogastrostomy (Endoscopic Ultrasound-Guided Cystogastrostomy) is a minimally invasive endoscopic procedure used to drain pancreatic fluid collections.

 

Indications :-

  • Symptomatic pancreatic pseudocysts (infection, pain, gastric outlet obstruction, biliary obstruction)
  • Walled-off pancreatic necrosis (WOPN)
  • Persistent or enlarging fluid collections despite conservative management

Procedure Steps

  • Endoscopic Ultrasound (EUS) Localization

    • An echoendoscope is used to visualize the fluid collection and assess its proximity to the stomach or duodenum.
  • Needle Aspiration & Contrast Injection

    • A fine-needle aspiration (FNA) is performed to confirm the presence of fluid, followed by contrast injection to outline the cyst.
  • Fistula Creation & Tract Dilation

    • A cystogastrostomy or cystoduodenostomy tract is created using electrocautery or a needle knife.
    • The tract is dilated with a balloon dilator to facilitate drainage.
  • Stent Placement

    • A lumen-apposing metal stent (LAMS) or double-pigtail plastic stents are placed to allow continuous drainage of the cyst into the stomach or duodenum.
